“ESWA and Navicare have a respectful and collaborative relationship”

The Senior Care Options (SCO) program coordinates and delivers all Medicare and Medicaid covered benefits to eligible seniors. The goal of the SCO is to enable enrollees to remain in the community. A Team approach to care includes the enrollee, family members, medical providers and community agencies. The SCO has contracted with Elder Services of Worcester to provide the necessary community services through social workers called Geriatric Support Services Coordinators. (GSSC)

SCO Eligibility Criteria

  • Age 65 or older
  • MassHealth Standard
  • If a Medicare beneficiary must have Parts A & B
  • Primary Care Physician (PCP) in the network

SCO Program Services

  • Case Management and home care services
  • 24 hour clinical access through the R.N. Care Manager
  • Prescription medications with no co-payments
  • Some over the counter medications prescribed by the PCP
  • Transportation for medical appointments
  • Dental care including dentures
  • Vision services – eyeglasses
  • Hearing aids
  • Mental health benefits

SCO Enrollment Process

  • Elder completes the enrollment form
  • MassHealth verifies eligibility
  • A GSSC or R.N. completes the assessment and initiates a service plan
  • Members can disenroll at any time, coverage maintains through the end of month

Elder Services is unable to choose a SCO Plan for you; however we will provide

you with information to help you to make an educated choice.
