in Care (TiC) program. This program aims to reduce emergency department (ED) visits by improving
efficiency and lowering costs through coaching, assessments and referrals, guiding patients to suitable
alternative health care settings and strengthening social connection. This Success Story highlights how
an interdisciplinary team of RN case managers and Transitions Coaches, trained in a care transitions
