Community Transition Liaison Program
The Community Transition Liaison Program (CTLP) supports any resident, age 22+, regardless of insurance, who is interested in receiving support and assistance to transition to the community. If interested in accessing CTLP services, the CTLP team will meet with the resident and/or their designated representative to explore the possibility of discharge and to identify any barriers that may impact the resident’s ability to successfully transfer to the community. The CTLP team will work with the resident and/or designated representative in connecting them to state programs and other local community supports with the goal of ensuring a successful discharge, while also working closely and collaborating with nursing facility staff to create a safe discharge plan.
